PLAYER & PARENT CODE OF CONDUCT


Instructions are below. Make sure to print clearly, and that we have your email address and cell phone number. Email & Text are the best methods for the club to communicate properly through out the year.

• Do not date any forms.
• Print and fill out U.S. Youth Soccer League Registration Form. Only fill out the information from your players name and down. Make sure to sign the medical release part at the bottom.
• Print and fill out the Club Futbol Registration Form.
• Print and fill out Player & Parent Code of Conduct.
• Print and fill out Training, Registration & Uniform Agreement.
• Make copy of your players birth certificate.
• Email a head shot photo of your player for players card. If you can not email electronic pic we will take one at one of the training sessions. A copy of your players birth certificate and digital picture are required in order to be eligible for games.
• Make payment on the contact page of our website or at a training session for the items requested on the dates specified otherwise late fees may apply.
• Watch your player develope and have fun!
