Parent/Guardian Consent Form: I do hereby expressly assume all of the risks of participating in the club’s games and/or related activities, including but not limited to physical contact and physical injuries. I agree to indemnify and hold harmless and club members and/or directors, including but not limited to any adjoining facilities and Club Futbol, from any and all claims, suits, or proceedings arising allegedly or in reality out of the acts or omission and participation of the undersigned in any related activity. I also agree to all rules and regulations of Club Futbol.

Payment Due with Registration: I understand the fee is due and payable before the pre-season practices begin. No player will be allowed to participate in game play if dues are outstanding. No refunds will be given once the session has begun. Should a player decide not to participate after the club has purchased equipment or registered the player, no refunds of those and related expenditures will be possible even if league play has not yet begun.

Consent for Medical Treatment (Minor): As the parent or legal guardian of the above player, I hereby give consent for emergency medical care prescribed by a duly licensed doctor of medicine or dentistry. This care may be given under whatever conditions are necessary to preserve the life, limb, or well-being of this minor, named above.

Uniform Policy: Jerseys must be turned in following the last game of the season, to be professionally washed and evaluated to be replaced depending on wear and tear and size changes. If you don’t turn your jersey in upon our request, the following season you will need to purchase whatever is missing or needs replaced due to wear and tear.

Photographic and Video Image Consent: I am consenting to the use of the registrants name and any photographic or video images of the registrant in marketing, advertising, and / or promotion of by Club Futbol and any of its affiliates.
